Notice Regarding Form 1095-C

Employer-Provided Health Insurance Offer and Coverage

Under the Paperwork Reduction Act and recent federal guidance, employers are no longer required to automatically furnish a paper copy of Form 1095-C to employees. Instead, Form 1095-C will be provided upon request.

What Is Form 1095-C?

Form 1095-C provides information about the health insurance coverage offered to you by your employer. This form is not required to file your federal income tax return, but it may be helpful for your personal records or for verifying health coverage.

How to Request a Paper Copy

If you would like to receive a paper copy of your Form 1095-C, you may submit a request using one of the following methods:

Please include your full name and current mailing address with your request.

Timing of Requests

  • Requests may be made at any time.

  • A paper copy will be furnished within 30 days of receipt of the request.

  • Requests received after the current tax year may still be fulfilled.
