P&A Additional Debit Card Request Form

If an employee would like a card for their dependent(s), cards can be requested by completing the attached and returning to the address/fax number on the form. By law only dependents over the age of 18 can be issued a debit card.

Response to Intervention Forms (RtI)

Student performance is influenced most by the quality of the interventions we deliver and how well we deliver them.  The RtI forms should be use for the process.

Sick Bank Request Form

All units use this form to request sick time.  Please fill out and submit to either your union president or the superintendent.  They will carry the process to the end for you.

Substitute Evaluation for Staff

Please use this form to evaluate your sub for the day.  This form needs to be turned in to the building secretary.

Work Related Incident Report

This form should be filled out ASAP and submitted to your principal or supervisor in the even you experience a work related injury.
